Pharmacy HMR Process

Community Pharmacies  on the Gold Coast can identify patients who may require a Home Medicine Review (HMR).

Community Pharmacies on the Gold Coast can use the QCPP " request a HMR " paperwork on behalf of their patient and send to the GP.

Once the HMR referral has been supplied from the GP. Community Pharmacies can contact Murdock Pharmacy Services on the Gold Coast to arrange the HMR.

Once the HMR has been completed the GP will "Action" a management plan and provide a copy to the Community Pharmacy.

Community Pharmacies are in an ideal position to follow up with their patients in the weeks or months after the HMR.

Community Pharmacies can charge and Admin fee to Murdock Pharmacy Services for arranging the HMR referral.
