Patients can request a Home Medicine Review from their GP if they are concerned with their medications. 

Why would a patient request a Home Medicine Review from their GP ?

- Concern of an Adverse Effect from their medication.

- Concern their medication is not working as intended.

- Patient has had a number of medication changes recently.

- Patient recently released from hospital with new medications.

- Patient is not managing their medications very well and may need help.

Please remember that this service is referred from your GP, it is your GP's responsibility to ensure eligibility criteria is met.
