What is a Home Medicine Review (HMR)
According to the National Prescribing Service (NPS) each year more than 140,000 Australians have to go to hospital with problems caused by their medications. In up to 69% of these patients the problem could have been avoided. The Home Medicine Review (HMR) process was introduced to reduce these medication incidents.
The HMR process is where a Consultant Pharmacist ( Accredited with the Australian Association of Consultant Pharmacists ), on request from your Doctor, will come to your home and assess all aspects of your medication regimen, to reduce the possibility of medication errors and ensure you are taking your medications correctly. The Consultant Pharmacist will then relay this information to your Doctor for discussion at your next visit to your Doctor.
Who should have a HMR ( Home Medicine Review ).
* Patient is a current Medicare or DVA cardholder
* Patient is living in the community setting.
* Patient is at risk of, or already experiencing medication misadventure.
* Patient's Doctor believes there is a identifiable clinical need for the HMR service.
