How to Refer a Patient for a HMR ?
STEP 1 : Identify the Eligible Patient
Use the Medicare Eligibility Criteria to identify Patients and discuss the HMR process with the patient.
STEP 2 : Send the Referral to Murdock Pharmacy Services
Contact Murdock Pharmacy Services on the Gold Coast to arrange sending the HMR referral via email or pickup from medical clinic.
Step 3 : Receive HMR report, Action the Management Plan, Claim the Medicare rebate.
