A credentialed pharmacist is a pharmacist who has undertaken additional training and has been recognised by the Pharmaceutical Society of Australia . The credentialed pharmacist will have the initials MPS CredPharm (MMR) after their name and B.Pharm qualification.
The extra training undertaken is to ensure they have the skill and knowledge to identify medication problems during the HMR interview and report any recommendations to a patients GP. The credentialed pharmacist will make an appointment to interview the patient in their home and during the HMR process assess all prescribed medications, over the counter medications and any herbal/vitamin supplements.
